Health Insurance Analysis:
- Copy of most recent billing statement.
- Copy of Schedule of Benefits page from master contract and/or certificate booklet.
- Census reflecting employees' and dependents dates of birth, gender, single, children or spouse & children (# of children), salary, home city and zip code, occupation.
- Corporate location (city and zip code).
- Employer contribution.
- Are there any serious on-going claims?
- What other Employee Benefit plans are in place such as Life, Dental or Long Term Disability?
- Would you prefer “pre-screened” rates for accuracy (involves filling out generic card for health information) or manual rates from census only quotes?
- Are there any classes of employees considered ineligible or excluded?
